Shipping
and Handling
* Self-addressed mailing labels are also available.
* When
shipping casts to the lab, we use Federal Express pre-paid, first class mail pre-paid, or UPS.
* Pick-Up and delivery
service is available in the Phoenix Metropolitan area. For further information, please contact us.
Production Time
Our production time is 5-7
working days from the time of arrival of the cast in the lab. There will be a 25% additional charge on all rush orders.
Cast Storage
Negative casts
are stored at the labortaroy for 2 months from the date that they are received for re-evaluations, adjustments, repairs, or
fabrication of addtional pairs of orthotics devices. Once the cast have been digitized they can be stored in the computer
for over 10 years.
Covers and liners
A variety of lining and covering materials are available.
Covers: Vinyl, Leather, Plastizote,
Suede, Poly-foam and Neoprene.
Linings: Poron, Poly-foam, Vylite, and Plastizote.
Accommodations Available
We offer a complete
line of accommodative pads and cut outs that you may need.
Warrany adjustment and Remaking
of devices
COL offers an unconditional lifetime
guarantee against normal use and wear on the orthotic device shell. There will be no charge for replacement. There is
an additional fee for adjustments and re-make requests if it does not coorelate with the orginal prescription order.
Any errors that were done by our laboratory will be changed and corrected at no additional charge to you.
Outgrow Program
The Outgrow Porgram covers patients up to 15 years of age, who can recieve one free pair of orthotics as needed for
outgrowth.
Repair Service
Our
lab has the expertise to repair, customize , or modify orthotics originally fabricated by other laboratories. Repair
is billed according to actual work performed.
